If you’re moving to Portugal, registering with the National Health Service (Serviço Nacional de Saúde, or SNS) is an important step in settling in.

The SNS provides public healthcare through health centers, hospitals and other medical facilities. Foreign residents who meet the requirements can use public healthcare services, including medical consultations, nursing care and hospital treatment.

Many newcomers aren’t sure how to get an SNS user number, which documents they need or whether they can register before they have a residence permit.

This guide explains how SNS registration works in Portugal in 2026 and who is eligible.

What Is the SNS in Portugal?

The Serviço Nacional de Saúde (SNS) is Portugal’s public healthcare system. It runs through a national network of public hospitals, local health centers and specialized medical facilities. Services include general consultations, emergency treatment, maternity care, vaccinations, nursing and specialist treatment when it’s medically needed.

The SNS is separate from Social Security (Segurança Social). Social Security mainly handles contributions, pensions and financial benefits, while the SNS provides public healthcare.

Who Can Register With the SNS in Portugal?

Portuguese citizens and foreign nationals who live in Portugal legally can generally register with the SNS. If you’re an international resident, eligibility depends on your residence situation and the healthcare rules.

Non-EU residents

If you’re a non-EU national with a valid Portuguese residence permit, you can generally register at a local health center. That includes many residents who moved here on visas such as the D7 Passive Income Visa, the D8 Digital Nomad Visa or the D2 Entrepreneur Visa. Bring your residence documents and any other ID the health center asks for.

EU citizens

EU citizens living in Portugal can use public healthcare under EU and Portuguese rules. Your entitlement may depend on whether you work here, are covered by another Member State or have established residence in Portugal. An EU pensioner whose healthcare costs are covered by another Member State, for example, may need to arrange an S1 document.

Foreigners without a residence permit

If you don’t have a residence permit yet, you may still be given an SNS user number when you use a public healthcare facility. Having the number doesn’t automatically mean you’re entitled to publicly funded care, though. Coverage depends on your residence status, the information registered and any legal exceptions.

What Is the SNS User Number (Número de Utente)?

The Número Nacional de Utente is the identification number used in Portugal’s public healthcare system. It lets healthcare providers identify you and keep your health records.

Portuguese citizens normally get the number automatically through their Citizen Card. Foreign nationals can get one at a public healthcare facility, such as a health center or hospital. You can ask for it even if you don’t need treatment right now.

Having an SNS number isn’t the same as being registered with a local health center, and it doesn’t by itself guarantee that the SNS will cover your medical costs.

How to Register With the SNS in Portugal

Most foreign residents register at their local health center (Centro de Saúde).

Step 1: Find Your Local Health Centre

Look for the one that covers your address. It’s best to register near your permanent Portuguese home. Not sure which one that is? Call SNS 24 and ask.

Step 2: Prepare Your Documents

Bring your ID, residence documents and your tax number in Portugal. What you need can vary by nationality and residence status.

Step 3: Visit the Health Centre

Ask the front desk to register you with the SNS. If you don’t have a Número de Utente yet, they can usually give you one on the spot.

4. Verify your details

Make sure your address, ID information and residence status are recorded correctly.

5. Ask about a family doctor

Once you’re registered, ask for a family doctor (Médico de Família). Health centers have limited capacity, so you may not get one straight away. You can usually still use SNS services in the meantime.

Documents Required for SNS Registration

For foreign residents, the following documents and details will be necessary:

DocumentPurpose
Passport or national ID cardConfirms your identity
Portuguese residence permitShows your residence status and healthcare entitlement
Portuguese NIFTax number needed for healthcare registration records
Portuguese residential addressShows where you live locally
Proof of healthcare entitlement, if it appliesSupports exemptions or coverage arrangements

Depending on your situation you may need other documents. EU pensioners, for example, may need proof of healthcare coverage arranged through another EU country.

If your residence permit is still being processed, ask your health center which documents it will accept in your case.

Can You Register With the SNS Before Receiving Your Residence Card?

Many foreigners who are moving to Portugal ask this. The key is the difference between getting an SNS user number and being eligible to have your healthcare costs covered by the SNS.

You may be given a user number when you use public healthcare services, even before your residence documents are finalized. Under the general registration rules, though, coverage depends on your ID, residence and other details being correctly linked to your patient record. Some exceptions apply under Portuguese law.

If you’re waiting for your residence card, check your position with your health center instead of assuming an SNS number gives you full coverage.

How Much Does SNS Registration Cost?

Getting an SNS user number and registering at a public health center are free. There’s no government fee for either.

That doesn’t mean every healthcare service is free for everyone, though. What’s covered depends on your eligibility, the services you receive and the rules that apply. If you hire a private provider to help with registration, they may charge their own service fee.

Can You Get a Family Doctor in Portugal?

Yes, but it depends on availability. If you’re registered with the SNS, you may be assigned a family doctor. A family doctor provides general care, manages ongoing health conditions and refers you to specialists when you need one.

Not every registered patient gets one right away, though. If none is available, your health center can tell you what other consultation options it offers.

SNS vs. Private Health Insurance in Portugal

Public healthcare and private health insurance do different jobs.

SNSPrivate health insurance
Public healthcare systemCoverage under a private insurance policy
Eligibility set by public healthcare rulesEligibility and benefits set by the insurance contract
Access through public health facilitiesAccess through participating private providers
Family doctor assignment depends on availabilityChoice of doctors depends on the policy and provider network
Registration doesn’t guarantee every cost is coveredDeductibles, exclusions and coverage limits may apply

Many international residents keep private health insurance alongside their SNS registration. It gives you more flexibility when booking consultations or using private hospitals.

For some residence visa applications, you may also need health or travel insurance before you have access to Portuguese public healthcare.

NISS, NIF and SNS: What Is the Difference?

Portugal has three identification numbers that newcomers often mix up.

The NIF is your Portuguese tax number, used for tax and financial matters. The NISS is your Social Security number, tied to contributions and social benefits. The SNS user number identifies you as a patient in Portugal’s public healthcare system.

The three numbers are separate, though you may need more than one for the same administrative procedure.

Can You Apply for NIF, NISS and SNS Numbers Together?

Portugal has introduced a combined identification service for eligible foreign nationals who have an address in Portugal. At participating Espaços Cidadão, qualifying applicants can request their NIF, NISS and SNS user number in a single process.

Eligibility covers some foreign nationals who have applied for or been granted residence authorization, EU citizens and people in specified international protection categories. As of September 2026, the service was available at 12 participating Espaços Cidadão.
